It’s taken five years, but NASA has finally released its report on the Columbia shuttle disaster. Unsurprisingly, it blames the equipment and sadly its recommendations for changes to that equipment come too late for any of the remaining shuttle missions.
NASA is taking some comfort that the Columbia crew probably only lived for about 40 seconds before being knocked unconscious. It is believed the Challenger crew was awake and aware during the entire fall into the ocean.
