NASA faults equipment in Columbia shuttle disaster

It’s taken five years, but NASA has finally released its report on the Colum­bia shut­tle dis­as­ter. Unsur­pris­ingly, it blames the equip­ment and sadly its rec­om­men­da­tions for changes to that equip­ment come too late for any of the remain­ing shut­tle missions.

NASA is tak­ing some com­fort that the Colum­bia crew prob­a­bly only lived for about 40 sec­onds before being knocked uncon­scious. It is believed the Chal­lenger crew was awake and aware dur­ing the entire fall into the ocean.
